Merlin isn’t overly surprised when he goes blind.
Something was always going to happen. At one point he might’ve thought destiny wouldn’t let it, but if destiny gave half a shit how hard it was to appease then Merlin wouldn’t wake up with more old pains and aches than Gaius every morning. He wouldn’t have to cover his scars up with a glamour full time, despite keeping most of his skin covered. No, Merlin stopped expecting slack about the time he first killed in the name of Albion and got whipped for his incompetence the next day.
Destiny will not accept blindness as an excuse for failure, so Merlin can’t afford to, either. His purpose remains. Arthur still needs protection. This changes nothing.

Placing the back of his palm to Merlin’s brow confirmed his suspicions and he almost hissed in sympathy at the heat sitting there. He was burning up. Without thinking, Arthur continued where Gaius had left off, grabbing the cloth and squeezing out the excess water before resting it on Merlin’s forehead. Maybe it wouldn’t do much but it made him feel better, knowing he was helping just a little.
