“It’s not fear I wish for him to know.” Saki traced his hand lower down the boy’s skin. Such an ugly green. Trust Yoshi to downgrade so drastically after failing the first time with Shen’s offspring. “I’ve spent the better part of a year hunting down his family. Last he opened his eyes, I’d gutted him so intensely that you’ve wasted a small fortune on healing him. No, it’d be too easy for you to associate my presence with world-ending, blood-curdling terror.”
He cupped his hand around one side of Leonardo's face. “I want you to force him to love me.”
Stockman didn’t blink. “Why can’t I do both?”
or, at the end of season 2, the Shredder decides to keep Leo and instead sends a message by hurling Splinter's comatose body through the window. This gives him about three months to break Leo down into something new, through a combination of sensory deprivation, sensory overload, and a whole lot of mindfuckery. He wants a loyal devotee, even if he has to cram love down Leo's throat until it's all he's capable of bearing.

"Oh, come on." Raph threw his head back in annoyance. But still, he complied, getting up.
"Sorry April, we gotta go!" Mikey cheerfully waved- but just before he could end the call, April spoke up.
"Why don't you just shop during the day tomorrow? Would be a lot easier..."
"Because then humans would see us." Mikey's smile went away to make way for a frown. "Duh."
"But..." April turned around to face them again. "You guys can go out in the open now."
"And?"

Summary
Due to an unknown circumstance, the Hamato Clan of dimension 12 find themselves stuck in dimension 18 with a very different set of turtles.
The Hamato Clan is, of course, skeptical at first. Who wouldn't be? But, they have to work with these new turtles if they ever wanted to get home. And maybe they'd get some free therapy along the way.
On the other hand, the turtles of Mad dogs find themselves mysteriously drawn to the Clan, maybe it's due to the fact they were their counterparts, maybe it was something entirely different. Whatever the case may be, these turtles needed help. And surprisingly, the Mad Dogs did too
Or
I throw the 12 turtles into the rise dimension and we have ourselves a fucking PARTY both a fun one and a pity one
And a plot, with villains.

Mikey knelt beside the hive, watching the bees flit in and out, their tiny bodies heavy with pollen. Their steady, purposeful movement soothed him, making him feel a little less adrift and uncertain.
“Hey, little dudes,” he whispered, a small smile tugging at his lips. “You guys are working hard today.”
The bees didn’t answer, of course, but Mikey liked to imagine they understood him.
Or, Mikey starts a beehive and talks with the friendly little bees when things get to be too much. A Farmhouse AU.

“Mikey?”
Michelangelo doesn’t answer, standing alone in the shadows. Leonardo moves to place a hand on his shoulder but pulls his hand back when the youngest screams in utter terror, his eyes wild and staring at the space around them. It’s a sound that haunts them all to their core, ringing over and over and Mikey keeps screaming and sobbing, begging for something they can’t understand. The fungi quickly latches onto his face, mushrooms digging into his eyes, across his shell -
“Mikey!” Raph yells but Mikey doesn’t hear them, lost in his own nightmare and unable to defend himself as the mushrooms consume his face completely. Instead, they can only watch helplessly as Michelangelo gets dragged into the darkness.~
A mysterious infestation of fungi in the sewer system of their old home causes the turtles to each become trapped in the own never-ending nightmares. Imprisoned in the dark caverns and the last one standing, Leo must battle his own fears if he stands a chance of saving his brothers before they are all consumed by their own terror.

TLDR: remember that 2012 episode with the fungus? I rewrote that for 2003, but much darker.

Summary
Out from his shoulder extended a red semi-transparent mass, tendons twisting amongst veins, throbbing in tandem to his own heaves. Roots dug into the space where its end met Leo’s skin. It clamped down tight enough that Leo’s flesh turned a pale off-white, and it sank in deeper every second that the structure didn’t dissipate. The arm didn’t look like the one Raph usually formed, a thin yet solid outer exterior, but like a jagged glove threatened to swallow Leo whole.
Leo took one breath, then another. His vision went blurry, one eyeline’s path crossing with another, until the only thing he could see was the pulsing prosthetic as it slowly tightened its crushing hold on the remaining shards of the fire alarm. It wasn’t that something was wrong, anymore. Leo was wrong.
And he wasn’t alone.
Or, in a mystic accident, Leo becomes the unwilling bearer of his brothers' ninpō. In the onslaught of three foreign presences in his mind, it's not easy to keep ahold of himself. It's all he can do not to suffocate under his own family's love as he struggles not to forget where his brothers end and where he begins.
Or, when, "I'm nothing without them" becomes, "I'm nothing but them."

Leonardo doesn't belong here, he's not crazy, all he did was down a bottle of aspirin and almost jump off a building, not that big a deal. He's not crazy so why should he be in a psychiatric unit?
While Leonardo comes to terms with the fact that he needs to recover and has to stay in a psychiatric unit/ward he meets 5 unique people who become his friends, and one to become his potential lover? Leo learns about others' mental struggles and his own.

Leonardo Hamato was not crazy. He wasn’t supposed to be here. Did he look like a crazy person? All he did was down a bottle of aspirin and then chicken out from jumping off a building. He was wishing he wasn’t a wuss and just took the leap, but nooooo he had to call his Uncle Hueso because he had second thoughts. Well now he was having third thoughts, and those thoughts were telling him he was stupid for not jumping.
