Penny is Tonys biological daughter, but Pepper is not her biological mother. Tony didn't find out she was his daughter until he found her as spidergirl, so she was raised by May but had gotten to know Tony for the two years before he died. MJ is also gender swapped in this.

Going backward in time was a little jarring.
Stumbling forward, Tony caught himself on the side of a building, before bending over and heaving a little. His stomach cramped painfully, a little unhappy apparently, about the trip.
“Ugh,” he moaned, happy to have not added any more vomit to the alleyway than was already full of it. “Jesus,” he muttered, pushing off from the wall and trying to get his head on straight.
Strange did warn him that time travel wasn’t just some easy peasy thing. That your body didn’t and wouldn’t like being hurtled through space and time.
But it was worth it, always worth it.
