Sans can't know about it. It's not that he doesn't trust him, no. It's just that Papyrus knows Sans wouldn't like the idea of his brother trying not to eat for a whole month.

She felt her soul being yanked backward, and so her body followed along. She fell on a patch of grass just near the bridge and didn't quite bother standing up.
The sound of the trousle of bones came near, and eventually, a tall skeleton was looking down at her. "HELLO HUMAN! YOU HAVE TO BE MORE CAREFUL, YOU MAY HAVE FALLEN THERE!" his voice beamed.
The skeleton struck an oddly heroic pose before continuing, "LUCKILY FOR YOU, I, THE GREAT PAPYRUS WAS NEARBY! NYEH HEH HEH!"

A voice, quieter than a whisper, louder than a thought, told her to do it. She pressed it delicately, breaking in a few lines on her otherwise flawless skin. At first, those three were white. She's done it a million times with a blunt boxcutter before, she knew how it went. The lines would turn red, then disappear before she blinked, always only showing themselves to be admired for a few seconds before being gone seemingly forever. They never dared break the skin.
Her body tensed up, her forearm welcomed the unfamiliar sensation. The white lines didn't stay white, they turned an unfamiliar red, a red that stayed for a while longer.

When Papyrus accidentally finds "weird lines" on Frisk's wrist, he decides to tell Sans. He seems far more worried than Papyrus expected.
Sans tries his best to help Frisk, but she tries to deny it as if it wasn't self-harm but a "small inconvenience". He doesn't buy that.

But lately, he has had strange dreams. At first, they were just weird. But lately, they were becoming more worrisome, nightmare-like even.
But Papyrus prefers to keep it (and this weird, depressing feeling) to himself. He does not want his nightmares to be an excuse for him being lazy! He can't lose his head over some silly dreams!
