Kaito and Aoko didn't really had to come out to a lot of people. Their parents somewhat knew everything. When they started school, they were already Kaito and Aoko to everyone.
Excepting themselves, they just had to tell one person, a kid their age with whom they sometimes played in the park. And the conversation was like: 'Kaito is a boy now, Aoko is a girl', followed by an 'okay'.
To be fair with them, they really didn't know there was something they would have to come out for.
So, when they began school, their park friend was in the same class, and they became real friends. And one day after school, a few years later, their friend said to them.
"I'm trans too."
Kaito and Aoko just looked blankly.
"What is 'trans'?"
It was her time to be confused.
"Really? You are transgender." The others just stared at her. "When you born, the doctors declare a gender. If your gender is different than that, you're trans."
"Oh. You're a girl then?"
"Yes."
"Great! We outnumber Kaito again!"
Kaito advanced. "Do you have a name you prefer?"
"Keiko."
"Oh, It's cute. I like it."
Bonus:
"So, who do you exchange with?"
The two kids looked at Aoko.
"Exchange?"
"Yeah, you know, like Kaito and Aoko."
"That's not how it works. You are trans by yourself, not with someone."
"Really? I always though that we switched bodies when we were too young to remember."
"What?" Kaito looked at her, "How would it works? My body is from the daughter of my parents, but my mind is from the son of yours?"
"Yes, basically."
"That's why you wanted my name?"
"Um... No?"
"Oh my god! It is!"
