Summary:

When a gangland shootout floods the Trauma Center at Hankuk Hospital with victims, ruthless Chief Baek Kang Hyuk and his brilliant resident Yang Jae-won brace for another battle against death. But the medical crisis turns into a personal nightmare when the surviving gangsters reveal Jae-won's hidden identity: he is Wakagashira, heir to the dangerous Tanaka Yakuza clan, and his dying father demands *only* his hands for the surgery. Forced to confront his bloody past under the betrayed gaze of his mentor, Jae-won enters the operating room not just as a doctor, but as an obedient son. As Kang Hyuk grapples with betrayal and the threat of gangsters in the hallway, one question echoes: How deep does the gentle resident's facade run, and what does the monumental tattoo hidden beneath his scrubs truly mean for the future of the Trauma Center?

Three gurneys burst through the automatic doors, pushed by pale-faced nurses and accompanied by a group of men with hardened expressions and expensive clothes now stained red. The metallic smell of blood instantly overpowered the scent of antiseptic. Gunshot wounds, deep stab wounds – this wasn't just medical trauma; it was an attack.

Baek Kang Hyuk, Chief of the Trauma Center, materialized in the center of the chaos like a commander on the battlefield. At 42, his face was etched with experience and an inherent roughness. His sharp eyes, which had seen too much in the sands of Sudan as a soldier-doctor, assessed the scene in an instant.

"Multiple GSWs! Prep ORs 3 and 4! START!" His voice, rough as sandpaper, cut through the noise without needing to shout. He unconsciously brushed his left forearm, where the intricate black wings of his "Black Wings" tattoo were partially visible beneath his rolled-up scrub sleeve. A superstitious relic from another life, from another war, where he believed that ink warded off bullets. Now, it was just a nervous habit. His gaze landed on Yang Jae-won, his resident. At just 28, Jae-won was already beside the most critical patient – a middle-aged man with a gaping, ominous chest wound near the heart. Jae-won moved with calm efficiency, his hands steady as he applied pressure, his voice soft but clear as he instructed the nurses. *Good kid,* Kang Hyuk thought, a fleeting flash of pride momentarily cutting through the tension. He'd plucked Jae-won from colorectal surgery and tedious Dr. Ha for exactly this reason: raw talent, ice-cold composure under pressure, and a mind that absorbed trauma protocols like a supercomputer. Jae-won was his bet, his number 1, the first resident of his fledgling Trauma Center that he personally molded.

Kang Hyuk rolled his scrub sleeves up a little further, the black wings becoming more visible. "Park! Pressure on that femoral bleed! Lee, portable ultrasound, NOW! Jae-won, status on the Thoracic Injury?"

Jae-won looked up. His face, always serene and polite, showed intense concentration. "Developing tension pneumothorax, Professor Baek. Needle decompression performed. BP unstable. Needs OR immediately."

"OR 3. With me," Kang Hyuk ordered, already turning towards the antiseptic scrub sinks. Jae-won followed him with quick, silent steps, his presence radiating a quiet competence that Kang Hyuk admired.

Suddenly, the energy in the room shifted. A tall man, wearing an expensive Italian suit now soaked with blood, detached himself from the group surrounding the injured man. His face was a mask of glacial fury, and the other injured men shrank under his gaze. His eyes swept over the doctors with disdain, finally landing like claws on Yang Jae-won. He spoke, his voice low but loaded with a dangerousness that silenced the medical bustle.

"No."

Everyone froze. Kang Hyuk turned slowly, his expression hardening like granite. "Excuse me?"

The man in the suit ignored Kang Hyuk, his stare fixed on Jae-won. "Him," he pointed a thick, accusing finger, "operates. Yang Jae-won-ssi. Only him."

Waves of confusion rippled through the staff. Jae-won himself went utterly still. The gentle, attentive resident seemed to petrify. His posture straightened almost imperceptibly, the softness in his eyes replaced by an icy neutrality Kang Hyuk had never seen before. It was unnerving.

"Who the hell are you to dictate who operates in *my* ER?" Kang Hyuk stepped forward, deliberately placing himself between the man and Jae-won. His military instincts, honed in zones far deadlier than any ER, flared. "I am Baek Kang Hyuk, Chief of Trauma. *I* decide who operates. This man needs immediate surgery, and my resident *will* assist me."

The suited man didn't flinch. A humorless smile touched his lips. "Professor Baek Kang Hyuk. We know who you are. A good soldier. But this," he gestured towards the unconscious man on the gurney, "is Tanaka-sama. His life is paramount. And Tanaka-sama demands the best. He demands *Jae-won-ssi*." He paused, his eyes returning to Jae-won, who remained motionless and silent. The next words fell like stones, spoken in rapid, fluent Japanese: **"Oyakata-sama wa anata ga shujutsu suru koto o nozomu. Kare no inochi wa anata no te ni aru, Wakagashira."** *(The Oyabun demands that you perform the surgery. His life is in your hands, Young Master.)*

The silence that followed was absolute. The hum of machines, the hurried footsteps, even the moans of the injured seemed to fade. Every eye turned to Yang Jae-won.

Kang Hyuk felt an icy knot form in his stomach. *Oyakata? Wakagashira?* The Japanese terms hit him like bullets. *Oyabun* – Boss, Godfather. *Wakagashira* – Second Head, Underboss, the heir apparent. His brilliant, gentle, impeccably polite resident? *Yakuza royalty?* The linguistic fluency – Japanese, Mandarin, Korean – suddenly gained a terrifying new context.

Jae-won closed his eyes for a fraction of a second. When he opened them, the warm familiarity was gone, replaced by a depth of weary resignation and glacial authority that seemed impossible for the 28-year-old Kang Hyuk knew. He replied in the same rapid Japanese, his fluency native, undeniable: **"Wakaru. Shujutsu-shitsu ni hairu. Daremo jama shinaide kure."** *(Understood. I will enter the operating room. Ensure no one interferes.)*

He turned to Kang Hyuk. The respectful deference was gone. Jae-won met his Professor's gaze directly, his eyes black, impenetrable pools of intensity. "Professor Baek. This patient requires immediate surgery. I will perform it. Alone." His Korean was precise, cutting.

Kang Hyuk was stunned. Roughness he handled well – he dished it out in spades. But this? This was a completely different person standing before him. The quiet scholar, the resident he nurtured, the one he *chose*... was giving orders. Revealing a past steeped in shadows Kang Hyuk could barely begin to comprehend. The tattoo on his own arm suddenly felt heavy, a childish talisman against a reality far more complex than stray bullets in a desert war.

"You think you're qualified to handle complex thoracic trauma alone, *Resident* Yang?" Kang Hyuk snarled, the title loaded with disbelief and a growing, protective anger – not just for the patient, but for the resident he *thought* he knew. "This isn't a fellowship exam! This is life or death!"

Jae-won didn't blink. "I am qualified, Professor. More than you understand at this moment. Every minute we waste reduces Tanaka-sama's chances." He gestured towards the suited man. "They will ensure there are no interruptions. Please release OR 3."

The suited man gave a curt nod, and two of his less injured subordinates positioned themselves in front of the OR doors, their postures making it clear they would enforce Jae-won's request.

Kang Hyuk looked from the ruthless gangster to his resident – no, to *this stranger*. He saw the unshakeable certainty in those eyes, the same focus Jae-won applied to mastering surgical techniques now directed at a completely different imperative. He saw the unconscious Oyabun bleeding out. The soldier-surgeon recognized a tactical situation he couldn't win by force. The teacher felt a revolt of betrayal and profound confusion.

He clenched his jaw, the muscle jumping. His own past whispered – the need to survive, the secrets carried. But this? This was an entirely different battlefield. With a short, furious nod towards the OR, he yielded. "Go. But God help you, Yang Jae-won, and him, if you fail."

Jae-won gave a slight, formal, empty bow of his head. "Understood, Professor." He turned, and the transformation was chilling. The resident's casual bearing evaporated, replaced by a contained, dangerously graceful poise. As he walked towards the OR doors, Kang Hyuk noticed something: even under the thin scrub top, Jae-won's back seemed... different. It wasn't just muscle; there was a texture, a shadow of something vast and complex hidden. When he raised his arms to push the doors, his sleeves rode up slightly, revealing not smooth skin, but the intricate edge of a dense, dark tattoo – the claws of a beast or the edges of monumental wings – hinting at a full-body masterpiece that covered far more. *The tattoo,* the thought cut through Kang Hyuk like a blade. Like those old Yakuza bosses wore. Not a symbol, but an entire story on his back. *The heir.*

The OR doors hissed shut behind Jae-won with finality, isolating him with the dying Oyabun and a single nurse selected by the suited man who entered with them. The main suited man – whom Kang Hyuk deduced was Sato – turned his icy gaze back to the Trauma Chief.

"He's good, isn't he?" Sato said, his voice a low rasp. "The Wakagashira. The best with his hands... both with a knife and a scalpel." A cruel smile touched his lips. "The current Oyabun is his father. Direct blood. When the Old Man passes, Jae-won-sama will take it all. That's why *only* he can touch the Oyabun now. It is the son's duty. And a test of loyalty." Sato stepped closer, his tobacco-tinged breath hot on Kang Hyuk's face. "You are a soldier, Baek-ssi. You understand hierarchy. You understand that some things are bigger than hospitals. Take good care of our Wakagashira here. He is... invaluable." The unspoken threat hung heavier in the air than any shout.

Kang Hyuk swallowed dryly. *Wakagashira. Heir. Oyabun's son.* The pieces clicked together with sinister finality. The impeccable linguistic fluency. The composure under pressure that went beyond medical training. The occasional aura of authority Kang Hyuk had chalked up to academic confidence. It was all blood heritage and a brutal world Jae-won was trying to leave behind. And that tattoo... Kang Hyuk imagined the masterpiece covering the young man's back, a permanent emblem of the destiny he clearly tried to deny, but which had caught up to him within his own sanctuary.

Hours dragged. Kang Hyuk led the other surgeries on autopilot, his mind spinning around OR 3. Every time he passed through the corridors, he saw Sato and his men, silent sentinels. He saw the closed door. He thought of Jae-won's skilled hands – hands that could save lives or take them with equal precision – moving over his own father's body.

Finally, the light above OR 3 went out. The door opened. Yang Jae-won stepped out, pulling off his surgical mask. He looked exhausted, but his posture still carried a remnant of that commanding bearing. His eyes found Sato's first.

"Stable," Jae-won announced in Japanese, his voice hoarse from tension and hours of intense concentration. "The bullet is out. Lung damage repaired. He's in ICU. The next 24 hours are critical, but the prognosis is good."

A collective sigh of relief went through the underlings. Sato bowed deeply. **"Arigatou gozaimasu, Wakagashira-sama."** *(Thank you very much, Young Master.)*

Jae-won merely nodded, avoiding his gaze. Then, he turned and his eyes collided with Baek Kang Hyuk's, who stood a few meters away, arms crossed, his face an impenetrable mask of anger and disillusionment.

Without a word, Jae-won started walking towards the doctors' locker room. Kang Hyuk followed him, his presence a silent storm. They entered the empty locker room. Jae-won went straight to his locker, starting to peel off the blood-stained scrub top.

"Jae-won," Kang Hyuk's voice echoed, low and dangerously controlled.

Jae-won stopped, but didn't turn. "Professor."

"Turn around. *Now*."

Jae-won hesitated for a second that felt like an eternity. Then, slowly, he turned. And before Kang Hyuk could say more, Jae-won raised his arms and pulled the scrub top over his head.

The air rushed out of Kang Hyuk's lungs.

Covering the entire expanse of Jae-won's back, from shoulders to waist, and wrapping partially around his flanks and upper arms, was a traditional black and grey irezumi tattoo, with accents of intense red. It wasn't just an image; it was an epic narrative inked on skin. A majestic, furious dragon dominated the composition, its meticulously detailed scales seeming to gleam under the fluorescent light, its sharp claws poised as if to rend flesh, stylized flames dancing around it. It was a masterpiece of pain and devotion, an inescapable symbol of affiliation, power, and the weight of succession. Between the shoulder blades, a stylized clan emblem – Jae-won's Yakuza family crest – marked him as heir. The tattoo was a second skin, an armor of ink that screamed Yang Jae-won's true identity.

"So it's true," Kang Hyuk spat, his voice trembling with fury. "Wakagashira. Yakuza Heir. Hiding under scrubs and polite smiles." He took a step forward. "Was it all a lie? All the effort, the talent... was it just a cover? A hobby for the prince while waiting to take the crime throne?"

Jae-won finally turned fully, his torso exposed, the dragon seeming to come alive with the tension in his muscles. His eyes, though weary, burned with a ferocity Kang Hyuk had never seen. "None of it was a lie, Professor," he replied, his voice firm but with a raw vulnerability underneath. "Medicine... saving lives... that is *real* for me. It's my escape. My redemption." He raised his hands, still stained with his father's blood. "These hands... want to heal, not kill."

"But they *can* kill, can't they?" Kang Hyuk advanced, invading Jae-won's space, his anger a living thing. "Sato implied it. 'Best with a knife'. You know how to fight. You are trained for violence."

"I am not what they want me to be!" Jae-won's outburst was visceral, echoing in the empty locker room. For the first time, raw emotion – despair, anger, fear – cracked his composure. "I was trained, yes. Since childhood. To protect the family. To survive in that world. But I *chose* this!" He pointed to the hanging scrub top. "I chose to leave! I chose to be a doctor! But blood always pulls you back." His shoulders slumped, the fury giving way to profound exhaustion. "Today... I had no choice. It was my father."

Kang Hyuk looked at the young man before him. The dragon on his back whispered of violence and an oppressive legacy. But in Jae-won's eyes, he saw the sheen of unshed tears, the genuine struggle. He saw the brilliant resident he had molded, trapped in a web of blood and obligation Kang Hyuk could barely comprehend.

"You endangered this entire hospital," Kang Hyuk said, his voice lower, but still cutting. "You lied to me. You used my trust."

"I tried to protect them! Protect *you*!," Jae-won insisted, his voice cracking. "If they knew who I really was... what my family represents... you would never have chosen me. Never given me this chance."

Kang Hyuk was silent for a long moment, studying the young man's face marked by internal warfare, the monumental tattoo that was both his cross and his armor. The silence hung heavy, laden with broken trust and painful truths.

"Get dressed," Kang Hyuk finally ordered, his voice hoarse. He turned to leave, stopping at the door. He didn't look back. "You're suspended. Until I decide if I can still trust a... *Wakagashira*... in my trauma center."

The locker room door clicked shut behind him, leaving Yang Jae-won alone with the dragon roaring across his back and the crushing weight of his two colliding worlds. The tears he'd held back for hours finally fell, silent, tracing paths down skin marked by a history he never wanted, but always carried. The heir of darkness, with the hands of a healer, caught in the limbo between blood duty and the desire for redemption.
