Plot is still mostly the same except Ancient (like Fuedel or may be Heian era or something) Japan and Modern Egypt.
-Egyptian Teenager Atem who lives with his Grandfather Saimun who runs a small gaming shop. Who lives in Domino City
And his dad Aknenkamun and well his mom (who we simply don’t see like how we don’t see Yugi’s dad)
Atem is given an ancient Puzzle box from Ancient time period in Japan's history Upon putting it together awakens the several thousand year of spirit of a young Emperor named Yugi.
His friends of course being the priests + Bakura of course and maybe fitting Kisara in there some where. Perhaps Kisara is Bakura's sister in this? (What if she's the one who needs a surgery like Serenity/Shizuka did)
I’m thinking Joey as Mahaad, perhaps Kisara little sister? With Mana being Tea, Tristan and Duke of course replace with Karim and Shada
Isis can take Mai’s spot… maybe not but you get idea
Seto would well be Seto- raised by single dad Aknadin who is cruel, manipulative, very strict and of course has very high standereds. Company of course still making military/ war stuff until Seto turned it around wanting to make peace not war.
Bakura- really actually a nice fellow and a huge closet gaming nerd- has ancient object from Japan with the spirit of a thief named Ryou (Ryou was a nice person but after thousands of years of imprisenment mixed with Zorcs influences has kinda warped his mind) No wants to be his friend because their afraid of him (of course Atem and Co make friends with him)
Duel Monsters of course would still be thing- though instead basing of a game Pegasus discovered while I Japan instead
Atem and Seto are actually cousins may or may not know it (their dads having a fallout at somepoint disagreeing and dropping communications)
Don’t know how the Ishtars would fit in with all this (as side from removing some of the priests and simply making them part of Atem’s group of friends idk.

For the items, instead of the eye of Horus maybe a yin yang symbol or perhaps a Tomoe symbol are on it.
the Millenium Puzzle would still be the Puzzle but maybe not a Pyramid design-
Designed kind of like one of those Ancient Japanese/Chinese Puzzle Boxes only well not made out of wood and wearable.

Ankh- errr can just be called Millenium Key and with more of a key look to it I guess
Rod again I suppose it could stay being called the mil.rod again jsut with a slightly different design
Scales stay scales
Mil. Ring name. Could look something more like a talisman (just wearable).
Possibly made out of something other then gold.
They could still easily be made from or with gold (along with well you know the sacrificed people)
The six priests, maybe if not Priests Six Samurai? or something else instead. Or well maybe 5 and the Solomon would be like his adviser or something like that instead.
But like still the same idea-
Items were made by slaughtering the people of some small village (plus some dark magic) .
Using magic to summon great beasts for protection.
Instead of three Egyptian gods could be like - Sun Goddess Amaterasu (Ra), Hachiman War God (Obelisk), and Raijin God of lightning (Slifer/Osiris).
This is just the base idea I have for it, still fond of the idea just been stuck on where to go with it.
