Chapter 1

The Hokage’s office was supposed to be a place of absolute authority. At twenty-five, Naruto Uzumaki had earned every inch of it. The stack of paperwork on his desk detailed the latest blueprints for the Shinobi Villages United headquarters, a massive academy reform bill, and the official exoneration files of the Uchiha clan.

He was a world leader. He was the hero of the Fourth Shinobi World War. He was an Omega who had shattered every glass ceiling the old council had ever tried to build above him.

But right now, curled up on the examination table in the Hokage Tower’s private medical wing, he felt like a terrified Genin again.

"What do you mean, positive?" Naruto choked out, his blue eyes nearly popping out of his head. He stared wildly at Sakura Haruno, who was looking at a glowing green diagnostic scroll with an expression that sat somewhere between clinical fascination and absolute homicidal rage.

"I mean exactly what the chakra signature says, Naruto," Sakura said, her voice dangerously calm as she rolled up the scroll with a snap. "You’re pregnant. Roughly four weeks."

"What! How is that even possible?!" Naruto scrambled backward on the table, waving his hands frantically. "I take my supplements every single day! I have an alarm set on my sealing scroll! Even if I... you know, sleep around occasionally, this isn't supposed to happen!"

"Those supplements are ninety-nine percent effective, you idiot!" Sakura snapped, her hand twitching as if she were desperately resisting the urge to funnel her legendary monster strength into a clipboard and hurl it at his head. "They aren't a flawless barrier, especially when you possess the most aggressive, fast-healing cellular structure on the planet! Kurama’s chakra probably burned right through the blockers the second your instincts spiked!

Inside Naruto’s mindscape, a massive orange fox shifted behind giant golden bars, letting out a heavy, rumbling snort of absolute amusement. 'Don't look at me, brat. I don't control your heat cycle.'

"Sakura, please, you gotta re-test me, it’s a fluke—"

"It is not a fluke!" Sakura marched over, looming over the desk of the most powerful man in the world like an angry deity. "As the Chief of the Hospital and a member of your advisory council, I need facts. Who is the father, Naruto? Because in about two months, Shino and Sai are going to smell a secondary Alpha pheromone all over the Hokage office, and I need to know whose head I'm supposed to hunt down."

Naruto swallowed hard, a sudden heat flushing over his cheeks. He rubbed the back of his neck, his eyes darting toward the window, looking anywhere but at his terrifyingly sharp best friend.

"I... well, see, it’s a funny story," Naruto mumbled, his voice dropping an octave. "You remember last month? When I went to the Village Hidden in the Springs for that international trade deal?"

Sakura crossed her arms, her green eyes narrowing. "Yes. Hinata said your diplomacy was flawless."

"Right, right! Flawless! But, uh, on the way back, I was super stressed out, and my guard detail was keeping their distance to let me breathe. So... I stopped at a little tavern in the Village Hidden in the Steam. Just for a quick drink. Totally incognito, cloak over my head, face hidden." Naruto squeezed his eyes shut. "And... I met someone."

Sakura’s jaw dropped. "You met someone. At a random bar in the Land of Hot Water."

"We talked for hours!" Naruto defended himself quickly, his hands flying up again. "I swear, Sakura! It wasn't just... a random thing! We sat in this quiet corner booth, and we just talked. About the past, about the changes in the village, about how tired we both were... it felt like the first time in years someone actually looked at me and not the Hokage hat. And then the tension just got super crazy, and my instincts went haywire, and we ended up at an inn, and—ow!"

A heavy medical binder slapped right against the top of Naruto’s blonde head.

"Sakura! Stop hitting me! I'm the Hokage!"

"I don't care if you're the Sage of Six Paths!" Sakura yelled, her face flushed with a mix of exasperation and protective worry. "You slept with a stranger in a rogue-adjacent territory while carrying the secrets of the entire Five Nations in your head?! Naruto, who was he?!"

Naruto rubbed the sore spot on his head, his lower lip puffing out.

He looked down at his lap, his expression softening into something incredibly complicated—a mix of deep longing, nostalgia, and a secret, profound warmth that he had been burying for a month.

He didn't tell Sakura that the "stranger" had a scent that smelled like ozone, winter storms, and the faint, unmistakable trace of old Uchiha charcoal. He didn't tell her that when those dark eyes had looked at him under the dim lights of the tavern, the childhood loneliness they had both shared had instantly bridged the eight-year gap between them.

"I can't tell you," Naruto whispered softly, his blue eyes suddenly serious, carrying the heavy weight of a leader protecting a secret. "But... he’s not a threat. I promise you that, Sakura."

Sakura stared at him for a long, silent moment. She took a deep breath, the anger slowly draining out of her, replaced by the sharp, rational mind that made her Naruto's closest confidante. She looked at his fiercely protective posture, and suddenly, a realization flickered in her eyes. Her breath hitched.

"Naruto..." she whispered, her voice dropping to a shocked murmur. "Is it...?"

Before she could finish the name, the heavy wooden door to the medical office clicked open. Shino Aburame stood in the doorway, his high collar obscuring his face and his dark sunglasses reflecting the room's light. His posture was perfectly rigid, the faint, low hum of his Kikaichū bugs always present in the background.

"Hokage-sama," Shino stated, his voice completely level. "The diplomatic convoy from the Hidden Mist has just arrived at the main gates. Hinata-sama is requesting your presence in the council chambers immediately."

Naruto instantly snapped back into leader-mode, throwing his white-and-red Hokage cloak over his shoulders and adjusting his hat to hide his flushed face. "Right! Thanks, Shino. Tell her I'm on my way."

As Shino bowed his head and stepped back into the shadows to alert Sai, Naruto turned back to Sakura, giving her a small, pleading look.

Sakura just sighed, rubbing her temples as a massive headache began to form. "Go. Run the world, Naruto. But you and I are having a very long talk tonight. And you better start figuring out how you're going to fit a baby bump behind that Hokage desk.”

The atmosphere in the private briefing room deep within the Hokage Tower was dense enough to cut with a kunai.

Naruto sat at the head of the table, his Hokage hat resting on the polished wood in front of him. To his right stood Sakura, her arms crossed, looking like a stern warden overseeing a high-security transport. Seated around the table were the pillars of his administration: Shikamaru , leaning back with a deeply exhausted expression; Hinata, poised and sharp in her formal diplomatic robes; Shino and Sai, standing like twin shadows near the door; and Kiba, who was currently letting Akamaru rest his massive chin on his lap.

"Alright, everyone," Naruto started, taking a deep breath and locking his hands together. He looked each of them in the eye. "I called this emergency meeting because something major has come up. It’s a personal matter, but because of my position, it affects the security of the village and our international standing. I wanted to tell you all directly before the rumor mill could even start."

Kiba chuckled, leaning forward. "What did you do now, Naruto? Did you accidentally sign a trade treaty with a bandit clan again?"

"No," Naruto said, his voice entirely serious. He looked down at his hands, then right back up, his blue eyes fierce and unwavering. "I’m pregnant."

Silence dropped over the room like a heavy lead weight.

Akamaru let out a low, confused whine. Kiba froze mid-laugh, his jaw dropping so low it looked unhinged. Shikamaru closed his eyes, pinching the bridge of his nose as if actively praying for a genjutsu to wake him up.

Sai blinked, pulling out a small notebook. "According to my books on human relationships, this usually implies a celebration is in order, though the lack of a prominent Alpha presence suggests—"

"Shut up, Sai," Sakura ordered smoothly.

"Naruto..." Hinata was the first to speak, her sharp, Alpha presence flaring just a fraction—not in anger, but in an instinctively protective wave that immediately blanketed the room. Her lavender eyes searched his face. "Are you alright? Is the baby healthy?"

"Yeah," Naruto softened, giving her a grateful smile. "Sakura checked me out. Everything is fine. Kurama is... well, Kurama thinks it's hilarious, but we're completely healthy."

"A pregnancy," Shino’s level voice cut through the tension from the corner. He adjusted his sunglasses. "Why is this a security concern? Because an unmated Omega Hokage carrying a child will invite political posturing from traditionalist factions in the Daimyō’s court. Furthermore, as your guard, Sai and I will need to adjust our patrol formations immediately. Your scent will begin to shift within the week, Naruto. If an outside threat notices a secondary Alpha pheromone on you, they will deduce you have an undocumented vulnerability."

"Shino’s right," Shikamaru sighed, finally opening his eyes. He looked at the paperwork on the table, his brilliant mind already spinning through a thousand defensive strategies. "What a drag... The Feudal Lords are going to have a field day with this. They're already terrified of the power you wield with the Shinobi UN. An 'unmated scandal' is exactly the kind of leverage the old-school politicians want to use to weaken your authority. We can manage the PR, but we need a name, Naruto. Who is the Alpha?

Every eye in the room locked onto Naruto.

Kiba leaned over the table, his sharp canine teeth showing. "Yeah, who’s the bastard? Give me a name, Naruto. My tracking squad will hunt him down across all Five Nations by sunrise. No one takes advantage of our Hokage and just walks away."

"Nobody took advantage of me, Kiba!" Naruto snapped, a bright flush creeping up his whiskers. "It was entirely mutual! But... I can't give you a name. Not yet."

Shikamaru’s eyes narrowed sharply. He looked from Naruto’s fiercely stubborn expression to Sakura’s tense, tightly crossed arms. As the smartest man in the world, it took Shikamaru exactly three seconds to connect the dots. The eight-year absence. The sudden, absolute secrecy. The protective stance Sakura was taking.

Shikamaru let out a massive, world-weary sigh and leaned back in his chair. "Oh, you've got to be kidding me. It's him, isn't it?"

Hinata’s breath hitched. She looked at Naruto, a profound, knowing understanding dawning on her face.

Kiba looked back and forth between them, completely lost. "Him? Who is 'him'? What are you guys talking about—"

"Kiba," Shino interrupted quietly, placing a gloved hand on his old teammate's shoulder. "Silence. The answer is obvious if you look at the timeline of Naruto's recent diplomatic trip near the borders of the Land of Hot Water."

Kiba blinked, his brain finally catching up. His eyes went wide as saucers. "Wait... The rogue? The Uchiha?!"

"He’s not a rogue," Naruto intervened, his voice dropping into a powerful, commanding tone that reminded everyone exactly why he wore the hat. The room went completely still. "Sasuke kept his word. For eight years, he stayed away while we tore down the old council, cleared his family's name, and rebuilt this world into something better. He didn't attack the village. He didn't interfere. He waited for me to deliver on my promise."

Naruto rested his hand flat against his stomach, his expression turning incredibly soft, yet fiercely brave.

"We crossed paths by accident last month. We talked. And... things happened. He doesn't even know about the baby yet. So for now, the world doesn't need to know the details. We adjust the guard schedules, we handle the Daimyō, and we protect this village like we always do. I’m not ashamed of this baby, and I’m not afraid of what the council or the other villages think of me. I am the Hokage. But I need my pack to stand with me."

Hinata stood up first, her Alpha posture commanding absolute respect as she bowed slightly to her leader. "The diplomatic corps is yours, Naruto. If any foreign ambassador tries to question your authority or your virtue, they will answer me."

"The ANBU will tighten the perimeter," Shino stated seamlessly, Sai nodding right alongside him. "No unauthorized scents will leave this room."

Kiba crossed his arms, a fiercely proud grin returning to his face. "Fine. I guess I won't maul him. But when he finally shows his face back in Konoha, I'll punch him in the jaw at least once for making you do paperwork with a baby bump."

Sakura finally let out a small, proud smile, stepping up beside Naruto's chair. "See? I told you they'd have your back, you idiot."

Naruto looked around at his friends, the heavy anxiety in his chest completely melting away, replaced by the warmth of the family he had built. "Thanks, guys. You have no idea what this means to me."

"Yeah, yeah, don't get mushy on us yet," Shikamaru muttered, already pulling out a fresh scroll and a brush. "We have about two months before you start showing, Hokage-sama. Let's get to work on a strategy.”

Notes:

Hey so, important things first, Naruto is hokage and and Omega, he so cool! I love him, now being an Omega doesn't change much most things remain the same as in the canon.

Now the changes! Sasuke doesn't return to the Village, he just tells Naruto to prove that he can change the system. So to change it only then will he think about coming back.

And also! Naruto was in team 7, but their bond isn't that obsessive YET! They still have it,the bond they made due to understanding of loneliness.

Oh! And itachi IS ALIVE and he's an Omega. Not too well but alive.

And! Naruto made a ninja UN (we will can it Shinobi Villages United, SVU) WHAT DO YOU THINK?!
