Ever since Kacchan-nii's quirk manifested, he would always find himself staring at those sparkles. Those little stars are so beautiful. He always knew how strong his older brother is. And now, partnered with his quirk, the realisation that the gap between the two became more obvious.
His niichan is strong, smart, charismatic, and has a bright future ahead of him. Izuku, on the other hand, is weak, useless and probably quirkless too. He can't help to become anxious about his future. He just.. don't want to be alone. He don't want to be left behind by his Kacchan-nii. He knew he is being selfish. But just thinking about a life wherein his niichan is not by his side is suffocating him.
Like now, he can hear Kacchan-nii's frantic shouting of his name. He's panicking. I don't know what exactly has happened. One moment, he's cheering for his brother's big pops and the next thing he knew, he's already on the floor with a bloody nose. He's getting dizzy and starting to lose his consciousness. He can feel his heart squeezed on the sight of niichan's teary face.
'Please, don't cry' is the last thing he thought before completely losing his conciousness.

Inko and Katsuki are currently waiting outside the Emergency Room. Inko can't help but cry while thinking of her baby's situation right now.
Also, Katsuki. He kept on holding Inko's hand and his gaze is locked at the door of the emergency room. He is not crying. He is just staring at the door, patiently waiting for Izuku to come out. For someone who is always loud, this Katsuki is a shock. She knew Katsuki is always soft towards Izuku or anything related to Izuku. She can already foresee an overprotective brother to his little brother. It's endearing, really. But at the same time, she did not wish her youngest baby to become sick and frail, along with the possibility that her baby might be quirkless.
'Why is this happening to my little Izu? This is just so cruel and sad.' just as Inko continues to cry.

Katsuki knew that he is smart for his age. That's why when he saw Izu suddenly collapsed with a bloody nose, he can feel that something is wrong. His baby brother, despite being frail, is not supposed to have a bloody nose. That is not normal, especially if he's just laughing before all of that fainting. He don't want to admit it, because he is always brave. He is not scared of anything. But seeing Izu on his arms feeling so lifeless is a nightmare. He don't want to experience that feelings again. He won't cry. He will be strong for Izu until this fucking door opens. He is Izu's older brother and he swears he will be strong for his twin.
When the lights of the emergency room are finally off, both mother and child stood on their feet, waiting for a doctor to come out. And they felt relieved once a doctor came out and announced that Izuku's already stable and he would be transferred to a normal room.
But they can subtly hear the lingering "however" and they knew it won't be good. "Mrs. Midoriya, I know this is sudden but we need to have a talk regarding Izuku's condition. Please follow me at my office. " is the doctor's following remarks.
"Ah, okay doctor. I'll follow you in a minute" Inko nervously answered back. She bent down to Katsuki's eye level to talk to his son. "Ne, Katsuki baby, I just need to talk to the doctor who examined Izuku, okay? Can you go to Izuku's room and stay with him for the meantime? I promise I'll return immediately. I just don't want Izuku to be alone, okay?" "Okay, Mom. I understand! We'll wait for you in Izu's room." "Thank you baby. Now just go with the nurse and be a good boy" she kissed her baby and went to the doctor's room.
Inko is now sitting on a chair in the doctor's office. She's nervous on what really happened to Izuku. The doctor then came out and sat on the chair facing Inko.
"Hello, Mrs. Midoriya, my name is Dr. Akane Minami. Nice to meet you" the doctor offered a handshake which Inko gladly accepted. "I'm Inko Midoriya, Izuku's mother. Likewise, doctor. I just wanna ask what exactly happened to Izuku?" "Right. It's nothing serious. Izuku has a mild concussion which caused the nosebleed to happen. I'm sure that the concussion is caused by his quirk".
The casual talk about Izuku having a quirk has left Inko surprised. "What are you saying doctor? A concussion caused by Izuku's quirk? But that's impossible because there are no signs of any quirk manifesting in Izuku and we almost though that he is...well, quirkless" which sadly replied by Inko.
"It's alright Mrs. Midoriya. I understand that no one can tell if it is really caused by a quirk but I'm also a quirk specialist so I also looked on it. You see, my quirk lets me see how a quirk really works. Just by looking at a person, I can pinpoint its properties and how it works. So I know for a fact that his quirk has caused this. Actually, his quirk has been with Izuku since he's a baby"
"What? Izuku already has a quirk when he's still a baby? But I don't understand. Why did we not see it at all?"
"And that's where the problem is Mrs. Midoriya. On how Izuku's quirk works. Izuku has a twin, right? Can you describe him, Mrs Midoriya? "
"Ah, yes. His name is Katsuki. Unlike Izuku, Katsuki has a healthy body and a powerful quirk. While Izuku, well he's a bit frail and tends to be sick almost every week. I really am worried about Izuku because of that. We already scheduled an appointment here because we wanted to know what's causing Izuku to be sick. Is it connected to that doctor? What exactly is Izuku's quirk? "
"Please, don't be shocked but I think Izuku's quirk is the ability of the user to give someone a boost in their health, body and quirk. He can make someone truly powerful both physically and his quirk."
"Oh my! What a great quirk! I know Izuku will be happy to hear that doctor." Then she realized that it still didn't explain why his baby is sick or has a concussion. "Wait doctor. Are there some drawbacks to this quirk?" she asked nervously.
"I'm afraid there is. In exchange to all the boost and power he is giving to someone is his own life force. He is slowly dying Mrs. Midoriya."
"W-what?".
At that moment, Inko felt his world has suddenly collapsed.
