Katsuki knows that Deku loves him. After dating for about a year into their debut as pro-heroes - which had been followed by a lifetime of knowing each other - it’s painfully obvious to just about everyone.
Yet when Deku turns to him and says “I love you,” his brain shuts down. It’s not supposed to be groundbreaking in any way, but for the love of god Katsuki just can’t say it back.
So if he couldn’t say it, then he would just have to show him.
