Summary
A world in which the King is Yellow is a fragment of the Ceaseless Watcher itself.
When Jonathan Sims and Martin Blackwood open the rift for the fears, they get pulled through the gap. Simultaneously, in Arkham Massachusets, in 1934, in a parallel world, a cult undertakes a ritual to pull an eldritch horror into being. The ritual fails at its original purpose, but instead it pulls a sliver of the King in Yellow and a pair of not-quite-humans through the gap. The King is bound to a book, while Jon and Martin are left free to build a life together in the 1900s.
Five years later, Arthur opens the book and loses his eyes to the King. While Arthur is struggling to come to terms with the destruction of his old life and the start of his new, much more terrifying existence, Jonathan Sims and Martin Blackwood find their domestic bliss interrupted by nightmares.

Summary
Kayne doesn’t send Arthur and John to the 13th century, but rather 21st century England, in an alternate world where there are no eldritch gods, but instead embodiments of fear. During their hunt for the Blackstone, Arthur and John encounter constant horrors... as per their usual.
Meanwhile, Jonathan Sims has noticed a recent increase in the frequency of first-hand statements given by survivors, and they have one thing in common—a strange man with yellow eyes.
