Summary
What if Rhaegar Targaryen survived the Battle of the Trident and fled? With news that the rebels are marching to King's Landing, he has his wife Elia and children, Rhaenys and Aegon, smuggled out of the city. All four of them flee to the Tower of Joy, where Rhaegar and Elia are reunited with their lover, Lyanna, who gives birth to Jaehaerys.
Rhaegar soon hears that Ned Stark has taken the throne instead of Robert, and that his father is dead—killed after attempting to blow up King's Landing with wildfire. Now, even lords who would have supported Rhaegar without question are uncertain. The consequences of his and Lyanna's elopement, with Elia’s acceptance, have made him and his lovers deeply unpopular.
On Elia’s advice, Rhaegar decides to open negotiations with King Eddard.

Summary
What if it was Lord Eddard Stark, not Robert Baratheon, who faced Prince Rhaegar Targaryen on the banks of the Trident?
Armed with Ice, Eddard Stark meets Prince Rhaegar in single combat amidst the chaos of battle. Steel sings, rivers run red, and in the end, it is Eddard’s blade that strikes true—shattering the rubies from Rhaegar’s breastplate, scattering them into legend. With the prince slain and the Targaryen host broken, the rebel armies rally around Eddard Stark who claims Kingship through right of Conquest. Gravely wounded from the duel, Ned cannot lead the charge to King’s Landing. The task falls to closest friend and brother in all but blood—Lord Robert Baratheon, his task is to secure the Capital and take the Targaryens prisoners. And the most importantly look for the newly anointed King's sister and Lord Robert's own betrothed which will lead him to a certain tower.

After the death of Prince Rhaegar at the Trident and the destruction of the royal host there, King Aerys II Targaryen died a week later from poisoning. His infant grandson, Aegon of House Targaryen, Sixth of His Name, King of the Andals, the Rhoynar, and the First Men, Lord of the Seven Kingdoms, and Protector of the Realm, ascended the Iron Throne. His mother, Princess Elia, assumed the regency and released Ser Jaime Lannister from his Kingsguard vows, marrying him to secure the support of Lord Tywin Lannister.
The Reach army, having marched from Storm’s End, joined the newly formed royal host. With a force of 52,000 men, they were able to defeat the rebel host at Maidenpool and forced them into a retreat. However, the royalists were also hard-pressed to continue their campaign and were only able to occupy the southern Riverlands. The Riverlords loyal to House Tully fled to the Twins, which had been occupied by Northerners following the betrayal and attainder of House Frey.
