After RE Vendetta/New York, Ada picked Leon up from rehab, and after that, he just never really left.
They decide to start trying to have a baby because what the fuck else do you do when you have 20 years of unresolved romantic tension and trauma bonding, and it doesn't go exactly as they'd planned (or more accurately, as they'd hoped).
