After losing Pepper to Extremis, Tony decides to get the shrapnel (and thus his arc reactor) removed-- but he wakes up as President of the United States.
Tony's heart surgery is the last thing he remembers, a worst-case amnesia scenario that leaves the country with a leader who doesn't remember the last year of office, the election, or his marriage to the First Lady, Natasha Romanoff Stark. The country is two days into a national tragedy that's still unfolding, a biological attack on Washington, D.C. that has Tony and his key advisors underground as they coordinate the response-- but nothing about his situation feels right.
