Moriarty was not inside Megaton when it blew up. Instead he was dealing with the shadiest of his contacts - the cannibals of Andale - far from the city, behind a cluster of boulders, when it erupted in flame. The world ending explosion threw him and the cannibals far away from their shelter, and if he hadn't landed in a river, in the shallows (but not too shallow), he wouldn't be presumed anything. Likewise, if the cannibals had not bled everywhere downstream, he would have become mirelurk chow. Arising from the water with a concussion, he wandered off into the debris cloud.
