Summary
Naruto suffers from a hereditary disease called dilated cardiomyopathy, a muscle heart disease where the ventricles enlarge, thin and weaken. In some cases a heart transplant is needed, but with almost a decade of waiting for a donor without luck he finds out he will die in half a year. With that knowledge he decided to not hold anything back and do the things he thought he would be able to do later on in the future, like confessing his love to none other than Sasuke. He does so anyways during there hangout and out of guilt and sadness that his best friend would die he accepts his confession and lies claiming to love him back. He promises to Naruto with the time he has left he will make him happy.
