Tommy has always been a horrific meddler.
He started at age 5 trying to run between their parents convincing one to apologize to the other, fabricating scenarios at any opportunity to get them alone together and somehow on a unified front. Somehow that front had always been “being upset with Joel.”
That hadn’t worked out all that well, but it didn’t deter Tommy from messing in other people’s business. He just wanted to help make people happy. If he got something out of it – all the better.

Dina's pregnant and Ellie's having a panic attack. Joel gets it.
“That’s good enough,” Joel reassures her, rough hands pushing stray hair off her forehead. “I almost passed out, when Sarah’s mom told me.”
Ellie takes another deep breath and huffs it out, wiping her cheeks, “doing better than you I guess.”
Joel chuckles, “you don’t have to tell my mother.”

It seemed reasonable, at the time, to assume that if Tommy could drag her over to show her the pups without incident, he could do the same for Ellie.
They hear Ellie’s exclamation first, excited chattering. Then silence. She sets their silverware out and imagines fondly that her husband is regaling her with observation and fun facts that could mostly be credible.
The idealistic version of it is shattered pretty quickly.
