It doesn't make Karlach smile like Lae'zel had anticipated, which bothers her far more than it should. Instead, the corners of her mouth twitch into a sad, bitter rictus of a grin and she says, "You'd be surprised what you learn to survive when you don't have anything left."
Lae'zel hadn't pitied her; Karlach was too ferocious a warrior for something as demeaning as pity. But she feels for her in a way she hadn't thought herself capable. In hindsight, it's this conversation more than anything that drives her to save the tieflings. A people whose creators were cruel, lacking the security and steadiness of a Queen's rule, of the love Mother Gith had felt for all her children.
(Later, Karlach will sit with her, and Lae'zel will say, quietly, "You are much wiser than I gave you credit for. You can learn to survive what you once thought unbearable."
"Aw, Lae," Karlach will say, and Lae'zel will bristle at the nickname and the way it makes her stomach clench not-unpleasantly. "You don't have nothing, you've got me." Lae'zel will fall a little in love then.)

The look on Karlach's face upon feeling the heat of Avernus on her skin dissuades her of the tentative notion of fortifying this place as a place for Karlach's engine to stabilize. It's hunted, haunted, like the animals in cages at the Circus of Last Days, like the rats scurrying about in the sewers, like the thousands of spawn in the depths of the Szarr Palace.
It's not a look that suits Karlach. Lae'zel will find a fix for her heart so she needn't wear it again.

Summary
Calling Karlach blunt does her a disservice, Shadowheart knows. Karlach is honest, certainly, almost to a fault, and emphatic with it. But blunt implies accidental harm and blind carelessness. Over their time together, both fighting their way to the Absolute and in this more recent month of peace, Shadowheart has seen nothing but care from Karlach. She burns with grace - not grace as in elegance, though there is an adroit smoothness to her movements, the artful skill of one who knows her body so well it goes beyond thought. No, Karlach's grace is in her aching compassion, her bone-deep kindness. Honest, genuine, earnest, yes. But not blunt.
Shadowheart reminds herself of this, attempts to batter the fact of it back into her skull, as she stares at Karlach openmouthed across the campfire.
“You want me to what?” she manages.
“Hurt me,” Karlach says.
(Karlach and Shadowheart have a discussion about sadism and trauma, and then they sleep together about it. It’s nice.)

Karlach likes Lae'zel. She really does... So the last thing she wants to do is fuck up their new relationship by harbouring secret mind-flayer fucking fantasies.
She knows the illithid are monsters. They're the sworn enemy of the gith for a reason. And there's a pretty high chance The Emperor is just manipulating all of them to get what it wants.
But they still haven't found a way to fix Karlach's hellfire engine of a heart and she hasn't touched anyone in a DECADE. After realising the engine doesn't work in her dreams with The Emperor... Well, the temptation gets harder and harder to resist.
