Aerion flashed him.
OR: Aerion’s demanding and Duncan’s desperate to save Egg (and himself) from a lifetime of embarrassment. They compromise. Somewhat, but not really.
Egg still suffers nonetheless.

“He’s got his daddy’s dome,” Aerion joked, though he was interrupted by a grunt, a new contraction firing up. His face pinched, but he still regarded Duncan with a look of quiet mirth.
“God can only hope he’ll put it to good use.”
OR: Aerion is good at everything. It’s a common fact. That even includes birthing babies, big babies, so long as there’s an incentive involved.
Duncan is in awe, and just slightly mortified by the fact.

“The horse’s hoof struck him directly against the temple,” The maester whispered. “The skull was fractured. We had to remove shards of bone to relieve the swelling of the brain. My Prince—if he wakes—the lad you knew will not return to you. The damage to the brain is irreversible, we gathered that he will not speak as he did and he will not think as he did. I have seen many men and women after injuries like this. Should he open his eyes, he will be—he will be as a sweet babe, childlike if the shards had not taken away his wits as well. He will need to be cared for, fed and clothed for the rest of his days just as an infant would.”
After an injury leaves Aerion Targaryen far different than before and entirely reliant on his family, Maekar Targaryen does his best to take care of his son.

Summary
In the aftermath of Ashford, everyone lives, with varying degree of injury. In the year since, Maekar has not spoken to Baelor, has not forgiven him for choosing a stranger over blood. He has emptied Summerhall of everyone he loves and called it penance. When the king forces a marriage to seal the crack, Maekar brings nothing with him but his silence. Baelor tries anyway.
