A hunting injury leads to a hospital stay for Sam courtesy of a bullet wound, and a gut-wrenching realization for Dean courtesy of the scars on Sam’s wrists. They’ve got to talk about this, except that Sam cries at hearing his nickname and Dean’s heart is in pieces in his chest, and neither of them are okay.
Dean wants them to get there so badly, if only he can just figure out how. If only he can just get Sam to look at him and talk to him.

The problem, Sam thinks, is that his cheek still really fucking hurts, and that’s kind of making it hard to search for snarky rejoinders to being punched in the face, or even to ask, hey, Dean, what the hell, I’m like two-thirds of your size and that really hurt, cause the part of Sam’s brain that’s normally focused on being quick and clever and whining at Dean is currently occupied telling him don’t ask him, you idiot, cause you don’t want him to hit you again.

It’s funny how Sam openly said that he was ready to die, multiple times, and Dean never bothered to ask, not even once... Why?
Nobody ever asked why Sam was so eager to go with Death. If they had, he would’ve pointed to Charlie’s burning pyre and a town covered in darkness as evidence to help them understand why he shouldn’t be alive.
The worst part is, Sam knows deep down his brother agrees with this, even if he won't ever say it out loud without the mark on his arm.
