Eight years following the Cairo Incident, the Speedwagon Foundation receives incredible intel that the remnants of DIO's cult, now led by Enya and Pucci, are taking refuge in Italy. Rumor has it, they've potentially allied with the mafia for mutual benefit. Avdol and Sherry are dispatched to Italy to finish them off once and for all, but little do they realize, that's exactly what their enemies anticipated.
