Izuku joins a pirate crew and finds himself endeared to the ship's surly captain.

“I didn’t dismiss you,” the captain says. Izuku blanches and turns to see him placing a leather bookmark between the pages of his book. He sets it aside and rises.
He moves toward the table, gesturing for Izuku to sit. He does so, stiffly, as deep crimson eyes follow his every move. Once seated, Captain Bakugou joins him. It’s then that Izuku notices how enticingly handsome he is. Sharp, stubble-lined jaw, piercing gaze, and spiky blond hair that he keeps on the shorter side—at least compared to Izuku’s own. His shoulders and chest are broad; the cut of his physique is much more noticeable here when stripped of his coat.

Did he just...is that Deku?! Katsuki squints, raising his sunglasses to his head to get a better look. He feels it all happen in slow motion as his eyes settle on the well-toned, lightly tanned man with a smattering of freckles across his body and face, wavy green hair atop his head. Naaah. That can’t be Deku. There’s no fucking way. This Adonis cannot be the same nerd he grew up with and tormented all those years ago, the same kid who followed him around like a lost puppy; always small and wiry, no matter the fitness tips Katsuki hurled at him.
But Katsuki knows better and he knows how aging works. It’s definitely Deku.
“Shit, he got hot,” he hears himself say. A few people snicker around him, but fuck it, he doesn’t care what strangers think. They all have eyes too, they must notice as well.

All this mingles in his mind when he has a run-in with a strange Quirk that can bring his fantasies to life and soon Izuku finds himself at the center of a love triangle—or more like a quadrangle—with three Kacchans vying for his affections, including the real one who suddenly seems jealous?! That earned him a few expletives and a devilish face emoji.

Kirishima’s upbeat tone was completely disharmonious with his words.
“Thanks a lot,” Katsuki grumbled.
“Hey, I don’t mean it like that! But when was the last time you had a day off?”

Summary
During a villain attack, Deku is hit with the mysterious gas-type Quirk, which makes the affected incredibly ill with a wasting sickness, depending on how in love they are with someone. The only known cure is for him to fall out of love with the object of his affection.
Eager to have his friend and hero partner back, Katsuki wants to help him heal, having no idea that he's the one Izuku loves. The heroes must work to track down the group of villains behind the attack and to determine their motivations while Deku's in the hospital.
Through his attempts to help Deku with his plight, Katsuki comes to realize his own feelings and he's desperate to have at least one moment with Deku before it's too late, and hopefully, the heroes will discover an alternate cure, so the two of them can be together.

It's a jumble of feelings, really. One minute Katsuki's being dragged into the hazy purple warp gate by that stitched-up corpse-looking fucker, while stupid Deku calls after him, and the next he's tumbling through the air as he barely makes his escape.
As much as he's loath to admit it, this close call with the League of Villains rattles him and forces him to confront himself, his relationship with Deku, and his drive to be the best. He'll be damned if he lets anything get in his way, even if that means himself.
