The several concussions Katsuki obtained across the years means he still has nasty migraines sometimes. Izuku decides to help by sharing the pain through their soul bond. In the end, they both bite off more than they can chew, but thankfully, their love for one another makes everything right -- that, and some chamomile tea.

Deku scoffs, pulling back slightly. "You underestimate me, sir." Scarred hands cup his face, and when Katsuki looks up, he sees a fierceness in those green eyes, burning bright with a determination that takes his breath away.
“Tell me, Kacchan—do I strike you as someone afraid to bleed?”
Katsuki lives alone in a small house in the woods. No one visits him, and no one bothers him, which is exactly how he likes it.
Then one day, a witch rolls into a nearby town and becomes its new apothecary.

He wonders what kind of person his betrothed must be. He's met Prince Izuku of the Eastern Kingdom all of a handful of times, all when they were barely old enough to walk. The memories he has of him are hazy at best: the afterimage of a bright smile and the impression of green. Nothing of him as a man, and the barest bones of him as a future ruler—kind, his mother says, a good head on his shoulders.
He'll be good for you, she'd told him. A complement. A balance.
I don't need balance, he'd scoffed. The idea of needing to rely on anyone to rule, much less a spoiled brat of a human prince who wouldn't know what it means to fight if his life depended on it, made his blood boil and his throat lick with flames.
The Eastern Kingdom hasn't seen wartime in a century, known now for its healers and scholars—a glaring opposition to his own people's ways of warriors and hunters. What need does he have for a symbol of peace?
