It's always obvious when a doll needs maintenance. Its movements become sluggish. It no longer shows enthusiasm when it receives a reward. It becomes harder and harder to get it to show any expression other than neutral. When these things start happening, there's no other way to fix it than to open it up and set things right. Clean out all the internals, scour away any gunk that might have built up. Check the runes inside the casing, redraw any that have begun to wear away. Polish everything to a mirror sheen, check that it all fits back together neatly, and rebuild your little friend, just as fresh and bright as the day it was made. Of course there are things that can be done to maximize the time between servicing, but sometimes there's nothing for it. They just need a good cleaning every now and then.
