This could actually be William either as a dentist or while still in high school getting ready to perform dentistry on another student.
This piece was made using the Mega Anime Avatar Maker by Rinmaru Games. I made some additions and changes using Microsoft Paint. I added the word balloon with William saying 'Open wide.' I also changed the pen William was holding to a dentist drill and added the cord (though I think maybe I should have made the cord a bit thicker). I also added the sound effect and spin motion. I also recoloured the hand so that William was wearing examination gloves and so that it was also showing the cuff of his dress shirt. I also added the buttons to his dress shirt.
