A hospital is not built in one day. The Polar Tang’s operation room and patient room took some time to be finished. Even with some inventions of Wolf and some gifted supplies from the clinic of Swallow Island, it was still not enough for Trafalgar Law.
In the North Blue, Marine ships and cargo ships would be missing boxes of medical supplies. IV fluids, gone. Syringes and needles, gone too. Medicines, gone. No one had any clue why medical supplies went missing for a while. It was blamed on the inventory clerks at first but as soon as expensive equipment began to disappear, they knew it had to be the work of a devil fruit user.
Law finds himself in a Marine ship carrying medical equipment to a Marine base in the Grandline. In the dimly lit room the medical apparatuses occupied, Law turns on his flashlight.
“Scan” Law views inside one of the crates,“ A ventilator? Lovely, the Polar Tang’s operation room was missing this. ROOM.” The blue sphere expands so it can reach the Polar Tang under the ship. Law switches the ventilator with a paperclip he had lying down on the floor. Law keeps lurking.
“Stop right there, pirate!” The door was slammed wide open with Marine guards. Most of them look young and inexperienced.
Law chuckles to himself. “So you saw Room and it brought you here?
“You can’t leave. We have you in this room with us!” The Marines point their guns at Law. “Tell us your name! You are under arrest.” Some seem to shake having to actually deal with a pirate.
“Trafalgar Law. However, I am just telling you this so you can tell my name. ROOM” A blue sphere engulfs the Marines as Law makes a hand gesture. They see ‘DEATH’ on Law’s hand. Law grabbed his sword and cut them
Many flinched or tried to fight but it felt nothing. No pain was felt when they knew Law made contact. They feel weightless. However, they felt like their body parts were dispersing. Limbs and head away from their torso.
“MY ARMS ARE CUT OFF”
“MY HEAD WAS SPLIT IN FOUR”
“WE SHOULD HAVE LET HIM LEAVE”
Law lets the pieces of the body merge into one. “I don’t kill. I am a doctor.” Law smirks and teleports to the Polar Tang.
“HELP US” A loud cry can be heard down halls and through vents. A mass of humans crawled out of the room. The lives of many young men were ruined that day. Who knows what happened to them as the World Government does not disclose how they take care of the victims of Trafalgar Law.
However, since that day the Doctor earned the title of “The Surgeon of Death”.

“Hey Captaaiin” Shachi smiles as he hides something behind his back.
“What?” Law says unamused. Shachi hands Law his wanted poster.
“You got a bounty and they are calling you the “Surgeon of Death’. Ooooo, scary. It is probably because of your tattoos.”
“Probably.” Law looks at his bounty and raises his eyebrows a bit. 80,000,000 Berries
“But how do they know you are a doctor? It's not like you are doing operations on the Marines.”
Law sighs. “It is probably because you, Penguin and Bepo tell everyone I’m a doctor. We always go through medical supplies when people line up outside the Polar Tang for medical help.”
“Oh well, you are right about that… You hear that, Bepo was calling me to help him sweap the sand, see ya Captain.” Shachi leaves before Law scolds him to not tell everyone he is a doctor. Although, Law loves announcing that he is one.
