Basically Isagi Yoichi died. Because of overworking/over exhausting his body, yep that's how isagi died. Well not quietly of course, dying in front of thousands of crowds in the bleachers and on the field is really....not that quiet.
Isagi Yoichi, currently standing on the field with a languid gaze he opens his tired, bagged eyes, seeing his entire team run up to him on the field. His body just laying there. Cold. On the field. He saw everyone's face darkened, some even looked like they saw a dead body. Oh wait they did. Isagi laughed bitterly. He officially overworked himself to death. He liked soccer wait- now scratch that he loved it. Soccer is everything to him so why?.
Why? Does it feel like a chore to him, now that he's dead...
Then he was given a chance...a chance to change...a chance to correct his mistakes...Will he take it?

Dr. Yoichi Isagi, a man of Zero Stars, who roasts patients until they wake up to reality. He get's assigned to the nation’s most notorious asylum, Blue Lock: home to lunatic prodigies. Armed with sarcasm, logic and questionable sanity, Isagi tries to survive sessions that feel more like boss battles than therapy.
Things begin to shift, between conspiracy boards, supernatural glitches, and patients who’d rather monologue about “meaning of life” than recover, reality itself starts to warp and Isagi can’t tell if he’s uncovering the asylum’s secrets… or just losing his own mind.
In place that feels too wrong to be real, Isagi walks in the hallways creaking with something he doesn't wants to see - Yet it's always watching him.

Yoichi looked around and found what he was looking for located around a pile of trash. Yoichi smiled, he quickly grabbed the ball and clutched it to his chest and quickly turned around to exit the alley- except something stopped him. What he thought was a pile of trash wasn't really trash.
Yoichi blinked once, then blinked again. Wanting to find out if his eyes were delusional because he thought he saw a human, specifically a child, camouflaged into trash.
Or... an unexpected meeting happens.

Basically Isagi wakes up one day in blue lock with bunny ears and a tail. Follow him on this adventure where he desperately tries to hide them from his teammates and gets fawned over in the process!!
(This was somewhat inspired by Cat-Boy isagi by @kiiiyosei in that it got me thinking about this trope!! But i thought instead of a cat boy id give isagi rabbit ears instead since his name is close to Usagi which is supposed to be rabbit or hare in Japanese)
