"Ideally, I want some unforeseen medical complication to tragically befall you, so you aren't my problem anymore." He admitted easily, because the hope had crossed his mind more then once. Shigaraki's eyes snapped open in shock, but the light made him hiss and squeeze them closed again before tears leaked. Izuku managed not to laugh as he continued. "More realistically, though, I'll settle for getting you healed up."
"Why?" Shigaraki all but whined. Izuku snorted.
"Because as long as you're injured, you're a patient, and thus under my care. But after that, you're just a bastard I can pawn off on actual heroes who get paid to deal with stuff like this." He explained, tone slow and condescending, because it was obvious.

or; someone finds Shigaraki in a gutter, sees his shoes, and turns him into the mostly-illegal quirkless clinic. Now, shortly after their friendly run in at the mall, he's offically Izuku's problem, and exactly no one is happy about it.
