When Ellie was leading David's men away from where she'd hidden Joel and the horse was shot out from under her, she fell.
Hard.
In ancient times, the first sign of developing civilization was a healed broken leg, as it leaves the individual all but helpless while they recover. And in David's words, "even lone wolves need to be cared for when they break their whole left side."
Little does he know that she's not a lone wolf.
Because Ellie’s got Joel. And he will never stop protecting her.
(Canon divergent from episode 8. When Ellie falls, she breaks her left arm, leg, and two ribs, leaving her vulnerable and trapped in the lodge as David insists on caring for her. Because of her injuries, she's not fit to fight - but Joel, despite his own stab wound, is willing to fight to the death to get his girl to safety and get her better.)

“I c… I can’t keep going on like this, Joel,” Ellie sniffled from the hiding place she’d made in his shirt. She was so tired of the nightmares, and the panic attacks, and not being able to eat without being sick; she’d been waiting for all of it to lessen with time, but it hadn’t. In fact, it’d only gotten worse. “... I’m so tired.” Her words shriveled up, and were soon accompanied by the now familiar sound of her crying. It was softer, more tired sounding than he’d grown used to.
“I know, baby girl,” Joel uttered, letting his breath fall warmly against the top of Ellie’s head as he sighed. If he could take all of her pain away and put it on himself, he’d do it in an instant, but he couldn’t, and so he was left feeling nothing but helpless. “Listen to me, things will get easier-”
