Summary
Neravna, "Nera" for convenience, is a mage.
Not the best thing to be born as in Runeterra, even if her arcane gifts aren't the flashiest. As an empath, she's more than aware of others feel - especially when it comes to their disdain for the mage-born. After exiling herself from Freljord and surviving a perilous journey to Zaun, she believes she's found a place as a club singer in the Underground - no magic needed.
It would be during a midnight performance when that would change. Midnight, when the Eye of Zaun shows up unannounced.
Nera should've known better than to fall into the enticing lull of security and safety she crafted for herself in Zaun, especially with a secret like hers.
And secrets always have a way of reaching the light.

Summary
You're pregnant with Optimus' sparkling. Though you're getting plenty of medical attention by Ratchet through the entire experience, you're very much lacking in attention from the one you want it from most: the sparklings father. After a revelation about how much he needs to work on his communication skills, Optimus more than makes up for it.
Could be read as a sequel to Tell Me How You Feel, though if it is then damn does Optimus have some issues with running away from his feelings.

The two go for a walk, fluff ensues, which leads to some nice sexy times on the beach to make everything better.
