Just as Jax, who has not told anyone about his trans identity (FTM), was beginning to develop a relationship with Pomni, he is horrified to find out that the genitals given are based off of biological sex. Even after finding out Pomni is also trans (MTF), he still fears to tell her the truth, and does his best to keep the secret for as long as possible in the fear of losing her.

“Jax. I know that this may be quite a difficult way to have woken up.”
Her voice is level. Not soft. Not sharp. Measured, practiced, like it does not need to negotiate its place in the room.
Jax does not respond right away.
The heart monitor beeps beside him, elevated and insistent, a reminder he’s still tethered to something. its rhythm syncing unpleasantly with the migraine still pulsing behind his eyes.
He swallows. His jaw tightens.
“…Just me?” he asks.
The question comes out quieter than he expects. Not hopeful. Not afraid. Braced.
The woman follows his line of sight briefly, her eyes passing over the cords, the headset on the floor, the mess he made. When she looks back at him, her expression has not changed.
“No,” she says. “You are not my only patient.”
That lands.
