Kara has decided to move back to Argo City, and Alex has come with her to say goodbye. Their goodbyes are interrupted as Alex meets with Alura, and Thara takes Kara to get updated on her vaccines.
At the hospital, Kara agrees to have an extra procedure. She did so reluctantly and without truly understanding what she agreed too. Now Kara has to have sex in the next few hours, or she will die. Specifically, she has to have sex with Alex, or she will die. Kara doesn't have a say in the matter because she's under sedation. So it is up to Alex to decide if Kara will live or die.
