Sam comes back from hell without a soul, and a sinful need for the angel who rescued him. Only Dean, and Cas' own misplaced sense of loyalty, stands in Sam's way of getting what he wants. So, without a moral compass to guide him, Sam decides to take what he wants, and no one will stop him. Not Dean. Not even Cas himself.
