Summary
With the threat of extinction looming over humanity, Taylor Hebert makes a final desperate play at victory against the indomitable force of a raging, golden god.
She succeeds while Scion dies. As the final loose end remaining, she is disposed of without further ado.
Left for dead, her usefulness concluded, Taylor's broken body drifts in a nameless void... until it lands upon a sunny beach on the edge of reality to be saved by a well-meaning witch and her assorted gaggle of demons, weirdos and strays.

Summary
Taylor is a horrifically bullied teenage girl who has the superpower to control bugs and is three steps from going Carrie on her school. The only thing keeping her going is a burning desire to become a hero, and have literally anything in her life not defined by trauma. So when an opportunity to become a hero arises she leaps for it, even if it means jumping into another world.
Ciri is a traumatized princess hunted by her past, filled with unfathomable interdimensional power, righteous determination, and less restraint than one might hope. Caught by a monster and about to die, she reaches out with her power to pull a hero from another world to save her. A hero and, maybe, a friend.
Geralt is a magically mutated superhuman who has been stoically exterminating monsters for over a century. He is good at his job, reliable, and avoids politics at all costs. Politics, however, sometimes finds him anyway, which is how he found himself the ward of a young dethroned princess with more bravado than wits. He'll manage just like always though, and find a way to balance his monster slaying job with caring for this magical teenage powerhouse.
However two teenage powerhouses might be too much even for him.

Summary
Some thought that a trigger event was the traumatic part of getting powers.
In her case, it was the "vision" that followed.
Having lived thousands of years through the eyes of a hundred and more legends from another world, all in mere seconds, chosen through naught but mere happenstance during the death throes of a far distant reality, she found herself the bearer of uncountable beasts and monsters, men and women, abilities and spells, all borne from fantasy, yet far too real to ignore or pretend they did not once exist.
In the aftermath, she starts to realize that no matter how much she tries, a normal life on Earth Bet is impossible, not with what she's become, what she can do. The people she can help.
Inevitably, the walls lengthen, the bars thicken, the shackles tighten.
Undaunted, beaten but unbowed, she resolves to drag Earth Bet to a brighter tomorrow, no matter how much it kicks and screams on the way there.
Whether she can succeed is another matter entirely.

Summary
An avid Star Wars enjoyer and otherwise average - a bit of a nerd and somewhat lazy - American guy wakes up as a Sith acolyte of prodigious size on Korriban. Beginning with the dubious instruction of Overseer Tremel, he must adjust to life in the Empire, and to the realities of a new body that challenges his expectations at every turn, hoping to survive long enough to become a power within the Sith Empire of legend.
Or, alternatively, Acolyte Xanot has spontaneous, massive memory loss on her way to Korriban and thinks she was once a basic-ass Human from another world.
