But even when Izuku was saved from the sludge villain, he still couldn't apologize.

Inko was a mess, and missing posters went up.

Katsuki looked for him constantly, even after getting into UA. He vowed to himself that he would never give up.
That was when he found him, he would apologize for everything.
What happens when chasing after a notorious super villain who was famous for destroying hero agencies, he got caught under flying debris but at the last second, he was tackled out of the way by the infamous masked villain.
