Yes, somewhere in the back of his mind, Shadow worries. Something is wrong with him, and he’s terrified that he won't be able to protect Maria when it’s imperative. It's like a nightmare playing in his head—running from something in the dark, not being fast enough despite super speed, not being strong enough despite super strength.
And it's teetering on the edge of his reality like a word on the tip of a tongue.

“What the—” he hissed, feeling the wet cold seep into his skin as his mind finally registered the icy bite of the morning. He descended the steps carefully, each step reminding him he was barefoot in the middle of a snow-covered yard. Squinting over the wooden fence, he finally spotted a figure bundled up in a hat with a large pom-pom on top. They were bent over, struggling to pry a stubborn lump of snow off the ground with a shovel.
Grinding his teeth, March called out, “Hey, kid! Go make snowmen somewhere else—it’s 6 a.m.!” The figure stopped mid-shovel and turned around, and March's annoyance melted into a groan of disbelief as he recognized her. Shanni, breathless from exertion, stared back at him, her cheeks flushed from the cold.
"You’ve got to be kidding me," she said, her tone dripping with sarcasm.

Summary
All he has to do is get through one simple check-up. Should be easy, right? But for Shadow, exam rooms and doctors are a traumatic reminder of the experimentation done to him aboard the ARK. Experimentation done without even asking him. And when that trauma rears its head in the middle of his exam, Shadow starts to lose his voice. Figuratively and literally.
