The Nara Clan were always in Konoha since before the beginning. There is history hidden away from view, so enveloped by the darkness that even Shikamaru only stumbled upon it on accident. The seals buried deep within the compound were so old, neither Shikaku nor Yoshina even knew they existed to begin with.
So when Shikamaru starts arguing with his shadow, they're confused and as his caretakers, start to get worried.
The Haruno household was far from being a clan, with Kizashi being the first shinobi in his family and Mebuki in a similar position. However well this may be, both were not strangers to the ways chakra could change the human body and mind.
Even with this in mind, neither could fathom the way their little Sakura was having arguments with herself.
