Summary: "Sheriff, I—I need your help."
That was never a good thing to hear over the phone at 2:48 in the morning, and the fact that it was Dean on the other end only made it worse. Of course, Jody wasn't going to turn him away. She couldn't imagine what kind of trouble he had managed to get into this time--though, knowing the Winchesters, it was most likely supernatural in nature--and why Dean was saying "I" instead of "we". She sure as hell wasn't prepared for what showed up on her doorstep four hours later.
Dean looked exhausted: dark circles under lack-luster eyes and several days' worth of beard over a tense jaw and grim mouth. He was alone except for the small, sleeping child he held bundled in his arms.
"Dean? Where is Sam?"
Dean, looking more lost than she'd ever seen him look, glanced down at the child in his arms; and he smiled even as tears filled his eyes.
"This *is* Sam."

Summary
For centuries, an ancient coven of devil-worshipping witches has preyed upon child victims for their sacrificial rites. Sam and Dean are after them, but as the investigation leads Dean in the wrong place at the wrong time, the coven makes sure neither he nor his brother will cause them any further trouble. They transform Dean back into his child self, leave him in the mountains in a small cardboard box in freezing temperatures and give Sam a choice: he can come for his brother, or he can chase the witches and leave him to die.
With no guarantee that his brother will ever be back to his old self, Sam takes Dean back to the motel and begins caring for him - but neither of them knows what to expect of the coming weeks.

When of the best Supernatural stories I have ever read. The writing is stunning and the story is superb. If you are going to read one Supernatural story, make it this one.

Their mother looked a bit like she’d taken a blow to the stomach.
“Dean?” she asked, voice hushed.
Sam heard a tiny sniffle and watched as another tear slid down Dean’s cheek. “I hate this,” he said, voice thick. “I can’t stop crying.”

After a hunt gone wrong, Sam is left to look after his much, much younger brother. It's not like he's taken care of children before.

The rain falls harder and Tony turns, his neck creaking and cracking, and he sees Peter asleep over by the window. He’s holding a small, flat box, and he’s slowly slipping to the right side of the easy chair he’s in.
Tony thinks about letting him sleep, but he finds himself speaking anyway. “Pete,” he says, his voice rough and raspy.
Peter immediately startles awake. “Happy Birthday,” he says, almost like he’d fallen asleep practicing it, planning to say it as soon as he woke up. He blinks at Tony, shivering a little bit, and then he smiles. “Happy Birthday. Happy Birthday.”
Tony snorts, smiling back. “Thanks, bud,” he says.

He frowned at hearing his name, still unsure what exactly was going on. He watched as his teacher continued to stand and stare out the door for a minute before seemingly remembering herself and taking a step back.
“Of course! If you could just go sit next to him until your turn, he’s in the back on the right side.”
The man stepped through the door and Peter gaped with the rest of the class as Tony Stark, in his signature suit and goatee, sporting a pair of red sunglasses and carrying a suitcase walked through the door.
