Vash hasn't always been in Nai's life, but since they've been reunited, their relationship has developed far past familial despite there being others in Vash's life. Others that Nai aren't too fond of and it makes it hard for Nai to believe that Vash even loves him.
Especially when he has sex with them also. That's a fact that Nai doesn't want to let go because it riles him up in more ways than one, even though Vash tries to get him to.

Summary
The boys made it through the night, their separation surgery sucessful. However, their mother fared worse, having passed due to complications with their birth. With their father out of the picture, the hospital immediately put them up for adoption. Quickly the paler boy, now named Nai so lovingly by his new parents, was adopted by a rich family. However, the other, called Vash by those who fostered him through the years, didn't have it so easy.

Quietly pulling Vash to him behind the main office table, Knives held his brother loosely by the hips. “You’ll have to recount all this to me when you wake up.”
His brother frowned with an uneasy smile. “...what?”
“You’re dreaming, dearest,” Knives persuaded, voice saccharine sweet.
“No, I’m not…!” Vash let out an anxious laugh, then his face morphed into worry when Knives kept on smiling at him. “Nai, what's going—”
