Summary
Death intimately recognizes the look on Zagreus' face. Haunted. Mortals bear that same expression in their final hour; their brows drawing taut with exhaustion; their mouth slacking in silent prayer; short, rattling wisps of life spilling from pursed lips; their eyes moving constantly, whether open or shut, trying to read the thread of their lives that the Fates had woven through an ever-expanding tapestry. It is a mercy when Thanatos takes them. And here Zagreus is, trying to find his thread in the middle of Elysium.
Zagreus has finally found his mother, but the joy in their reunion is quickly tarnished by the threat of Olympic wrath. With all the lies he's had to tell and all the people he's involved in its web, the weight of it all is crushing him alive as he struggles to hold everything, including himself, together. Thanatos refuses to let him carry the burden alone.

Back then, they stood on opposite sides of a gulf: Zelda resenting the confines of her childhood, Link trying to pick up the shattered pieces of his. Bridging the gap now means building upon grief and unfamiliarity. But she’s spent so long missing him—she’s sick to death of wasted time.
“I’m right here,” she says, sliding her fingers through his. “Will you tell me what happened?”
Fireplace shadows flicker over his bandaged face. Maybe he’ll refuse. Zelda remembers Epona’s hooves kicking up dust, his proud green tunic disappearing behind the crest of a hill. Maybe all hope for their future died that day—or maybe time and tragedy only helped it grow.
Link’s answer comes slowly, like drawing venom from a wound. “Well,” he murmurs, tightening his grip on her hand, “I got lost.”
The hero searches for something he’ll never regain, the princess becomes a queen, and fate gives them a second chance.

Summary

A year has passed since the Phantom Thieves struck down a god and saved Tokyo from a terrible fate. But when people all over the globe begin to suffer from "Paranoia Syndrome," mysterious seizures which leave them contorted and unconscious, Akira Kurusu finds himself in the Velvet Room once more, demanding answers for the affliction. His investigation leads him to the Dead Sea, the rotted remnants of the cognitive world, where all things go to be forgotten - including someone the Thieves had thought to be long gone.
Meanwhile, Akira's comatose body is found in the waking world, and his terrified friends suspect the worst. Akira's confidants struggle to learn the truth behind Paranoia Syndrome, while he and Goro Akechi approach a fated confrontation in the decaying shell of the cognitive Tokyo. As the sickness continues to spread, the Phantom Thieves must reunite and destroy the evil festering beneath the Dead Sea's waters, before its malignant influence unravels civilization forever.

Summary
Thanatos considers calling for Nyx. Surely, Zagreus must be ill, if he’s not only sleeping but thrashing about as if he’s in pain—and Thanatos is a reaper of souls, a dealer of death, not a healer. Yet he spots something from the corner of his eye that gives him pause.
The blanket is tented down at the level of Zagreus’s hips.
Startled, Thanatos simply stares for a moment, until Zagreus’s whimper transforms into a moan, and then comes that voice again. The needy, agonized tone that drew Thanatos to this chamber in the first place. "Than.”
Oh.
