Kinda got inspo from my fave fanfic Author (The_Negat1ve_One) yes let's get into it
John "Soap" MacTavish had faced death multiple times before, growling defiantly with glaring eyes at the barrel of a gun pointed directly at him. But after the encounter of Vladimir Makarov almost killing him, he felt like a man changed for the worst.
He didn't know what else to feel but confusion, shame, and disappointment.

“Do you have urges to act on thoughts of killing yourself?” The thought made John stiffen a little, he definitely had moments where he’d been caught by some soldiers in his squadron with a gun to his head. Being a soldier was difficult, mentally and physically taxing when you’d seen too much shit. The human mind wasn’t meant for that.
“In the past.” John mumbles out, grinding his teeth slightly, trying to focus on his bouncing knee instead of how his chest tightened slightly. A jolt of adrenaline at the memories. The ones with a gun, thoughts snarling in his mind on how John was nothing better but an animal for killing people.

Basically where John "Soap" MacTavish is in The TortureChamberVerse, he's being ground up, (aka brainwashed and having amnesia, never quite remembering his close or distant faces). Every place feels vaguely familiar, but at the same time, nothing ever felt the same. Vladimir appears further in the book for the sake of the plot.
John didn't know what was going on.
