Summary
When Sam and Cas enter Dean’s mind to try and save him from Alternate Michael, they discover that Dean’s hiding in a place Michael can’t reach him. In order to reach Dean, they must obtain the six pieces of his heart. But each piece is guarded, and Michael is still on the loose. On their quest through Dean’s psyche, Sam learns a few surprising things about his brother, and about Castiel.

"I know," Castiel murmurs, still just smiling a little, not looking heartbroken at all. "Don't worry about it, Dean, that's all I ask."
"So we're—" Dean risks a glance up, swallowing thickly. "We're good?"
Castiel hums. "We're good."

Now picture Dean stuck in that rom-com cliché—with Castiel.
Because that's what happening to him; a crazy whirlwind of your typical-and-not-so-typical cliché's. He's playing the main lead in all of them and Castiel's his counterpart. Of course, the culprit's obvious—Gabriel's enjoying himself, lying back on his favorite cloud with a bucket of popcorn.
