Jim has spent a long time and a lot of mental energy thinking things through, and from what he can tell, it goes like this.
A good man is a dead man.
A good man is noble, a martyr, a statue to be placed on the mantle and adored. A good man lives in photographs and memories. He smiles at Jim from the holo on his mothers dresser, he smiles from across the backyard as he tosses Jim the ball, smiles as he kisses Jim’s forehead and says-
“It’ll be alright, Jimmy. Just get out of here.”
A good man shows up two weeks later, trussed and hung in the middle of town square.
Jim has spent a long time and a lot of mental energy thinking things through, but he never had to think long about one thing.

Summary
Spock is dying. There is an oath McCoy took once, an oath that defines the very essence of his being. First, do no harm. It drives him, compels him; defines him. Now he has broken it, and to break that oath is to betray his innermost self. McCoy himself has hurt Spock, possibly beyond repair, and his world has turned upside-down.
