Yamada slowly pulled himself to his feet to step closer, gangly legs wobbling under the unsteady weight of himself. He could see her better, then; the dark mole under her left eye, her perfectly applied mascara, the faintest hint of smile lines at the corners of her mouth. Not a detail absent.
“I’m just how you picture your conscience,” she told him, and he sneered a bit, perhaps at himself. Don’t people usually dream about their teeth crumbling or something when they’re nervous? Why did she have to be here?
“So my conscience has Kayama’s face. What’s up with that?”__
Hizashi Yamada gets a stern talking-to, because someone has to reel him in, and of course it would be Kayama.

“You think someone’s trying to—?” Nemuri says, then covers her mouth with both hands. “Oh, God.”
“It is… not outside the realm of possibility,” Tsukauchi says, and again he directs that horribly sympathetic look in Shouta’s direction, “that Present Mic was taken by someone who intends to use his Quirk for their own purposes. Whether that involves the kind of work Doctor Garaki was doing remains to be seen, but… as of now, it can’t be ruled out.”

Summary
Hizashi relapses in the aftermath of Nemuri's death. Shouta finds him, and realizes the road to recovery is further than he'd thought.
Or, my short take on the ways Hizashi deals with his grief, having gone unsupported and buried down under his tough exterior as Hizashi, or his energetic Present Mic persona, never really processing his emotions until he finally breaks. Shouta is there to take care of him in ways he failed to before.
