Consequences. Part one: Tony dies of an allergic reaction or a reaction to meds when Fury has Natasha inject him with the compound supposed to help mitigate the palladium poisoning. JARVIS kills Fury and Natasha, razes SHIELD, then goes around wiping out anyone or thing he thinks deserves it.
Part two: Thor accidently breaks Tony's neck, killing him, while holding him up by the neck and threatening him about using the scepter. JARVIS painfully reassembles himself after being all but destroyed by Ultron. Discovering what happened, he destroys Ultron, apparently kills all the Avengers, and manages to kill Thor slowly and painfully. Tony's death is announced, and Loki, in the guise of Odin, retrieves Thor's body, and he and Pepper negotiate a sort of mutual non-aggression pact.
Part three: Wanda kills Tony at the airport. Rhode and Pepper take down the Avengers.
Part four: Steve kills Tony with his shield in Tony's workshop. Hulk kills Wanda, and finishes creating the android. Steve tries to defend himself, unsuccessfully, and a tiny bit of him wonders if he really did want Tony dead.

Eventually, everything comes full circle.
