He clenches his hands into fists. He clenches his jaw harder, because the howl of despair and misunderstanding is about to break. Because what the hell is going on here?! I wanted to scream obscenities, suddenly hit this once-beloved face, just to erase this strange smile, thin and neat, too perfect. It's too sharp, and you'll cut yourself a little more. And Dean cuts himself on it. Yes, he can't help but look. He can't help but try to figure out what kind of familiar stranger is standing in front of him.

But it was not this that confused Anaxagoras, but the fact that this same student entered the classroom without embarrassment, as if he had been here before. He smiled at the others, and then looked at him, with those same damned eyes. And Anaxagoras had seemingly never seen so much relief and happiness in anyone. Perhaps only in the eyes of his sister and his old mentor.

“Still… I'm sure y-you'd rather be stuck with any Time Lord other than me,” the Master replies.
There is the beginning of a smile forming on the Doctor's lips as he shakes his head, and a faint glimmer in his eyes that could be easily mistaken by the tears gathering there, but that in reality is caused by something else – or rather a collection of somethings; memories of a childhood shared and promises made.
“There's no one else I'd rather be with right now.”
The Doctor takes care of the Master after he gets gravely injured during one of their trips, and their relationship grows as they end up taking care of each other.

Summary
The last thing the Doctor expected when he was escorted to the Master's room was to find him in this state.
(Edit from 2025: Did some pretty substantial, fixed typos and updated the prose to sound a bit less like it was written by a 14 year old (which it was). Tried to tone down some of the less tasteful depictions of mental illness, as well as descriptions of drunkenness written by someone who had never been drunk in their life.)
