Along with Rex she goes to the colony Polis Massa where Padme gives birth and eventually dies. Leaving seventeen year old Ahsoka with the responsibility to raise said children. However Bail and Bretha Organa step in to raise Leia. Ahsoka decides that she must raise Luke.

Ahsoka Tano is a child dealing with major grief and loss and is thrust into another problem on top of all of that where her brothers wife is pregnant and dying, leaving her with the choice to walk away or raise the child. She decides to raise Luke.

She briefly registers cold concrete beneath her before peeling her eyes open. Glancing around with a bleary gaze, she slowly lifts her head. She’s pretty sure she has permanent brain damage. Being able to lean back on her elbows, a fuzzy view of – bars? – is suddenly in front of her. This has her scrambling back until she hits a wall, chest starting to heave. Fuck fuck fuck fuck fuck. She barely has time to think before a door on the far side of the room is opening, a familiar face strolling through.

Realistically he wouldn’t survive without bad injury’s so in this story he does Survive the main characters don’t think he’s going to he’s on the verge of life or death for a little while but he does eventually survive but has to undergo a lengthy recovery.
He never fully recovers due to how bad his injury’s are.
Like in the show in this he is deaf on his right side.

“Shhhh, baby,” Joel interrupted, his voice gentle and soothing. “I gotcha. You can sleep here all you want. I’ll keep you safe, Ellie. Promise.”
OR: After 7 years David is released from prison and is plotting to take Ellie away. Joel would rather die then let that happen.

"You slip through the wall, and you get Ellie the fuck out of here. I will find you after." Her lip curls into a snarl, fury and irritation chafing her expression. He knows that she's pissed, but they ain't smuggling partners on a run gone sideways right now.
They're parents.
"We don't get to leave, Joel. That was the fucking deal." And it still is. He's trying to get them all out alive.
That means she needs to run, and he needs to handle the fucking hunters opening fire in the direction of their family.
"I ain't leavin' and neither are you; this is just temporary." Hoping like hell he ain't lying to her, he tightens the strap of Ellie's sling and insures she's pressed tight against Tess.
He'll get back to them, and he'll mow down every fucker that tries to get in his way.
