A loud bang startled him as his door slammed open. Izuku turned around to see his door blown off its hinges and an irate Katsuki standing inside it.
"What the fuck did you just say to me? I should stop pretending like I care about you? I love you, goddammit. I spent the last eight years funding a suit so you can live your dream. And I built an entire agency so we could work together again. You rejected me, asshole. And then you ran off with Round Face. My fucking damaged heart has been yours and you stomped on it so if anyone's pretending to care, it's you!" Katsuki's chest was heaving and he had a wild look in his eye that Izuku hadn't seen in a long time.

Or, Katsuki starts hanging out with Monoma instead of Izuku after their last class reunion and Izuku's not taking it well.

Katsuki thrived on challenges.
He enjoyed the rush of adrenaline that came with them, the praise that followed.
The more difficult the challenge, the more satisfactory the feeling of overcoming it.
This one, however, just pissed him off. He couldn’t wait to see those sparkling green eyes swallowed by the waves.Or, 5 times siren Katsuki gives it everything in his power to try and get this insufferable mortal to drown, and 1 time he ends up saving him instead.

Summary
Nausea threatens him, either caused by morning sickness or anxiety; it was hard to say, but nonetheless, Izuku makes a distressed chirp, hoping to summon his alpha if calling said alpha's name wasn't enough. After a few more attempts and still nothing, that's when the stinging begins to build up behind his eyes, with the tightening of his throat thwarting further successful chirps. With one more sad attempt, Izuku starts the arduous task of getting to his feet, where he, once again, normally has Katsuki here to assist. The thought of that sees the first tear rolling down Izuku's cheek, and he twists and grunts until he's vertical.
If his alpha isn't going to come to him, then Izuku's going to have to find Katsuki himself.
