Summary
Castiel's Grace is kept safely inside Dean's chest, wrapped around his soul.
Dean grabs an insane Castiel out of the Empty. They have to make their arduous way through all of the after-life, in order to get back. Dean, is struggling to keep them alive and safe, all the while trying to come to terms with his feelings for the angel. It doesn't help that he has to fight everything and anything in between. Also, Castiel seems utterly unhelpful, unable to take his own Grace back from Dean.

A little while later, Dean considered what Cas said, while he was waiting for Castiel to shower in their motel room. He thought the former angel must be some kind of saint, being so casual about all of this. About Dean not being into Cas, but Cas being very much into Dean. He took it in a stride. While Dean did decidedly not take it in a stride. He was confused as hell. He kept thinking that Castiel wants to hold hands with Dean. And slow dance. And all that. Maybe even kiss Dean. Somewhere other than his cheek. He wanted all that with Dean. And he didn’t get it. And he was not bitter at all about it.
Cas was a better man. Dean would not have his patience. His selfless dedication. His cool attitude to rejection. If Dean was in love with Castiel... and Castiel was not interested. He would be mad. Hurt. Burn something down. Get into some fights. Burn some more things down, maybe? Anyway, not casual. He would yearn for Cas. For his chapped, delicate, smiling lips. -

Summary
Six months after Cas was swallowed by the Empty, Dean, Sam and Jack find a spell to pull an angel from the Empty, but the last ingredient is missing. Determined to save Cas, Dean travels back to 2009 to get the missing ingredient from the only person who can give it to him: Castiel, fresh from having pulled Dean from hell.
(Inspired by the tumblr post about what would be the most insane combination of Dean and Cas to meet. My answer was post-confession Dean and season 4 Cas)
