Summary
Samira and Jack had sex at medical conference in New York almost four years ago. They did not exchange last names, or indeed any other pertinent biographical information.
Dr Samira Mohan, intern, has a three year old daughter.
Unsurprisingly, these two facts are connected.

Summary
After leaving PTMC, Samira Mohan works to rebuild her confidence in medicine while facing personal struggles that led her to step away. She and Jack Abbott remain in casual contact through exchanging medical journals until Samira’s replies grow sporadic, and Jack learns she has joined Doctors Without Borders. Simara neglects to share that she is stationed at a hospital in a high-risk area nearest to an active conflict zone in Syria.
That is, until nearly seven months later, he is CC in an email from Doctors Without Borders along with other PTMC staff who might be willing to donate their time and the administrative staff to request the start of a program for young Syrian women with high needs care after brutalization from war, and who are seeking asylum. Jack sends two emails. One back in the PTMC group to confirm his support and time. Then another with just two words-
Syria, really?
The Samira Mohan who shows back up at PTMC three months later with a van full of girls isn't the one who left.

Summary
“Though they both spent so much time away from this place, twelve-hour shifts eating away at their days, returning to their bedroom always felt like a homecoming to Jack. It had technically been his before it had ever been theirs, but before that point, it had only been a place to sleep, and only when he had to. Now it was safe and warm, and it smelled like mint, jasmine, and slight hints of citrus. The darkest bits of the forest and the clean, aromatic scents of a garden.
In this room, it smelled like home.”
a.k.a. Dr. Jack Abbot comes home to Dr. Samira Mohan.

Summary
John Shen doesn't believe in coincidences. He believes in patterns.
He also believes, very firmly, that when two highly competent doctors who don't usually share shifts suddenly started overlapping at two in the morning, it is not a scheduling anomaly.
It is a situation.
And Shen, unfortunately for everyone involved, is very good at identifying situations.
